Understanding Obsessive-Compulsive Disorder

Obsessive-Compulsive Disorder (OCD) is a chronic and often debilitating mental health condition characterized by uncontrollable, recurring thoughts (obsessions) and behaviors (compulsions) that an individual feels the urge to repeat over and over. These symptoms can interfere significantly with all aspects of life, including work, school, and personal relationships. While OCD is often misunderstood or trivialized in popular culture, for those living with the condition, it is a source of profound distress and anxiety. Finding effective treatment is essential for managing symptoms and reclaiming a sense of control and well-being. At Psyche Denver, we recognize the unique challenges faced by individuals with OCD and are dedicated to providing specialized, evidence-based care that addresses the underlying biological and psychological components of the disorder. By answering the most common questions about OCD and our treatment philosophy, we aim to provide clarity and support for those seeking a path toward relief. What Are the Core Symptoms of OCD and How Is It Formally Diagnosed?

The core symptoms of OCD are divided into two main categories: obsessions and compulsions. Obsessions are persistent and intrusive thoughts, images, or urges that cause significant anxiety or distress. Common themes include fears of contamination, a need for symmetry or order, and aggressive or forbidden thoughts. Compulsions are repetitive behaviors or mental acts that an individual feels driven to perform in response to an obsession or according to rigid rules. These acts are intended to reduce the anxiety associated with the obsession or to prevent a dreaded event from occurring, although they are often not realistically connected to the situation. Formal diagnosis involves a comprehensive clinical assessment by a mental health professional, who will evaluate the presence of obsessions and compulsions, the amount of time spent on these behaviors (usually at least one hour per day), and the level of distress or impairment they cause. At Psyche Denver, our diagnostic process is thorough and empathetic, ensuring that we understand the specific nature of an individual’s symptoms and how they impact their daily life, which is the first step in developing an effective treatment plan.

Why Do Some Individuals with OCD Not Respond to Traditional Treatments?

Traditional treatments for OCD typically involve a combination of Selective Serotonin Reuptake Inhibitors (SSRIs) and specialized psychotherapy, such as Exposure and Response Prevention (ERP). While these interventions are effective for many, a significant portion of individuals—up to 40%—may not achieve adequate symptom relief or may find the side effects of medications difficult to tolerate. The reasons for this lack of response are complex and can involve genetic factors, differences in brain structure and function, and the presence of co-occurring conditions like depression or anxiety. In some cases, the neural circuits involved in OCD, particularly those in the orbitofrontal cortex and basal ganglia, may be deeply ingrained and resistant to standard chemical interventions. When traditional methods fail to provide sufficient relief, it often indicates a need for a more specialized or advanced approach that targets different neurotransmitter systems or utilizes neuromodulation techniques. At Psyche Denver, we specialize in identifying these treatment-resistant cases and offering innovative alternatives that can provide new hope for those who have struggled to find relief through conventional means.

Is TMS an Option for Treating OCD?

Transcranial Magnetic Stimulation (TMS) has been FDA-cleared for the treatment of obsessive-compulsive disorder (OCD) using specialized treatment protocols available on certain TMS systems. While research has shown promising results for appropriately selected patients, not every TMS device currently on the market is capable of delivering these FDA-cleared OCD protocols. In addition, insurance coverage for OCD-specific TMS treatment may vary depending on an individual’s plan and clinical circumstances.

At this time, Psyche Denver does not offer TMS treatment for OCD. However, our team is happy to discuss whether TMS may be appropriate for your situation and can help direct patients to treatment centers that currently provide FDA-cleared OCD TMS protocols when appropriate. We also offer comprehensive psychiatric evaluations and evidence-based medication management for individuals living with OCD.

What Should a Patient Expect During Their Treatment Journey at Psyche Denver?

A patient’s journey at Psyche Denver begins with a compassionate and detailed consultation to understand their unique experience with OCD and their previous treatment history. Throughout the process, patients are supported by a team of experts who provide close monitoring and ongoing assessment of progress.

We believe in a collaborative approach, where the patient is an active partner in their care, and we prioritize clear communication and education about every aspect of treatment. By combining the latest scientific advancements with a deeply human-centered approach, Psyche Denver provides a supportive and effective pathway for those seeking to overcome the challenges of OCD and achieve lasting mental wellness.

I had a difficult time finding a Psychiatrist that was accepting new patients after moving here from Chicago. Psyche was the first practice to return my call and I'm so happy they did. I have seen two different providers (both under the supervision of Dr. Richmond) and they have both been excellent. The thing that makes this practice outstanding, though, is the support staff/admins. They have a super convenient patient portal and always answer messages the same day, usually within an hour or two (shoutout Yoxelin! Although I am sure they are all wonderful). They have been able to help me with numerous medication issues, like when there's a national shortage and I need my script sent to 12 pharmacies until one can fill it, etc. I've never had such reliable access to my psychiatrists' care teams before and let me tell you, it makes all the difference in the world. I no longer worry that I will have to go without meds because I can't get ahold of my doctor. Very happy with the treatment I've received at Psyche and can confidently recommend.
